• Over 70% of female and 8% of male general practitioners admitted they never use a chaperone when conducting an intimate examination of patients.
When the general practitioners did use a chaperone, the sources were varied, such as:
• 22% of interviewed general practitioners used general practice registrars or students as chaperones.
• 47% used a family member or a person accompanying the patient as chaperones.
• 43% of general practitioners used non-clinical practice staff as chaperones.
• 78% of general practitioners normally used qualified female nurses as chaperones.
The main reasons that the general practitioners gave for not using a licensed and qualified professional medical chaperone for chaperoning in general practice are:
• Availability
• Confidentiality
• Time constraints
• Doctor-Patient relationship
Importance of Chaperoning in General Medical Practice
The use of a chaperone during an intimate examination, especially of female patients, is an essential step towards building a stronger, healthier and more transparent relationship between the general practitioner (doctor/medical professional) and their patients.
These are the top reasons that detail the importance of having a chaperone present during an intimate medical examination of a patient:
• Protection - The presence of a chaperone during an intimate medical examination adds a protective layer for the doctor against false allegations of inappropriate behaviour/sexual harassment.
• Trust-building - Chaperones are a way of acknowledging the vulnerability that the patient might feel for their dignity during an intimate physical examination.
• Assistance - Chaperones are also trained to efficiently help the general practitioner for performing examinations.

Technological Advancement in Health Communication in Less Developed Countries

Direct communication in health care can take place via forms of social media such as Skype, zoom and video conferencing. Traditional forms of communication such as telephone, notes and letters can continue to be used for information sharing between health providers and patients. For example, doctors and patients can text and send messages to one another. It is imperative that social media messaging be used to reduce the time taken to obtain knowledge on patient condition and general wellbeing.
The use of social media communication tools should not make indirect communication obsolete. In instances of technology resistance and lack of access to social media, health providers will be required to communicate face to face with patients and their relatives. Sometimes it may be easier and faster to communicate with patients via other media such as third parties. Doctors and patients can communicate via third parties such as nurses, personal caregivers, family and relatives of patients who may be unable to communicate. In other instances, it may be advantageous for family or caregivers to inform patients of their condition. A spouse or parent may be empathetic in conveying negative news to patients.
As a small island developing nation, Trinidad and Tobago faces serious health challenges that can be minimized by appropriate investment in social media technology. The major challenges include shortage of ambulances, bed shortages, and acute shortage of highly specialized medical knowledge and practice. Three additional acute problems are hospital overcrowding, limited availability of biomedical technology and drug shortages. These problems are exacerbated by traffic congestion that results from poor road infrastructure. Health and infrastructural challenges often combine to impact the quality of healthcare for patients with limited access to health facilities negatively.
This paper proposes that the quality of patient care for less critically ill patients can be enhanced by the adoption of social media tools that will enable doctors and other health providers to see and hear their patients in remote sites such as specialized care facilities and patient residences. The main intention of social media usage is to reduce overcrowding, improve access to healthcare, promote effective pain management and reduce patient death or mortality. Many patients can be released from hospital and be cared for at convalescent and private homes where providers can monitor and evaluate their progress via Skype, video conferencing, zoom or other appropriate technology.
The state can promote technological advancement and innovation to make social media tools widely accessible, highly reliable and very efficient. Investment in innovation should lead to the introduction of local media technology that can enhance the phenomenon of multiple users at the same time. For instance, doctors, pharmacists, nurses and other health providers should be able to interface simultaneously in order to ensure patients receive the best care. The doctor will maintain the primary provider role but should be able to take advice from other providers who may spend more communication time with patients. Finally after health providers have attained a position of consensus on the way forward, patients and their relatives can be brought in to ensure that the agreed treatment plan is explained and understood. The patient will become the biggest beneficiary of a collaborative approach among health providers. He will receive faster, more efficient care from a multidisciplinary team of healthcare providers.
Technology should enable patient surveillance in a virtual setting that should reduce the time and hazards of taking weak and incapacitated patients to a hospital or care facility. In addition, providers can give safe and efficient care to patients who are in remote locations. It also means that patients warded at hospital would benefit from more direct contact with health providers who would have more time to spend with them. One advantage that cannot be underscored is the learning that will accrue from evidence based practice. Health professionals can learn from one another and develop innovative collaborative approaches to healthcare. Patients will receive consistent instruction or advice from health professionals. Use of social media technology in health care will also enable patients to have sessions with health providers individually. The use of electronic health records will enhance patient care tremendously. Health providers will be able to access patient information faster and easier thus reducing the time taken to care for patients.
Technological advancement must be accompanied by cultural change. The biggest change, technology acceptance, should ensure that resistance to technology is reduced. Change must bring about significant gains in patient education in order that they become more responsible for their overall health. Patient empowerment programs must be implemented to provide greater patient self efficacy. In other words, patients must be able to diagnose partially, monitor and evaluate changes in their physical and mental health. They must be able to detect significant health changes and be able to relate them to factors such as diet, changes in their physical environment and general increases in emotional, mental and psychological stress. The overarching goal is to enable doctors to make faster, more accurate, diagnoses of patient health problems.
This paper recognizes the limitation of social media technology such as technological failure or malfunction,. However, this challenge can be overcome by putting the required personnel in place to respond quickly to it. More importantly, this paper purports that systemic problems such as bed and ambulance shortages will be reduced significantly by the use of social media technology to care for patients who do not require hospitalization or critical care.
In many countries patients complain about long wait times in hospitals. This paper suggests that the use of social media tools can improve the quality of healthcare, save time and lives. They can promote a collaborative approach among different health providers that will benefit patients and promote greater efficiency in the health system.

Chronic Fatigue Syndrome - How My Healthy Lifestyle Changes Helped Extreme Fatigue and Weakness

For 20 years, I lived with a great mystery in my life. I was diagnosed with Chronic Fatigue Syndrome (CFS/ME) and nobody could tell me why I had it or why it made me feel the way it did.
It was back in 1996, when my life completely changed. I was a busy, energetic woman who liked to keep on the go. I was rather casual about how I ate, what personal care lotions and potions I applied to my skin and hair, and after four years as a student in the USA, my general way of living was less than ideal. It was during the winter that I was hit with a bad case of flu. While others around me recovered, the constant fatigue and weakness that comes hand in hand with the virus, never completely left me. When it came to physical or mental exertion, I had no stamina and would have to take to my bed nearly every day to get through to the evening.
The symptoms were not only restricted to extreme fatigue. My eyesight would judder, like a TV programme with a poor signal, I would feel dizzy and have to sit down or I'd fall down, my heart would beat at 90 miles an hour just from the strain of waking up in the morning, and it felt like the signal that went from my brain to move my legs, was just not transmitting correctly. If I came in contact with any chemicals, even just to smell them, I would feel very unwell. I had bad IBS (Irritable Bowel Syndrome), my periods became erratic and my concentration was appalling. It seemed that every function in my body was out of kilter.
After two years of relentless tests it appeared that everything that could be tested was 'normal'. As a result, I received a diagnosis of M.E or, as it is more commonly known today, Chronic Fatigue Syndrome (CFS/ME). Diagnosis of CFS happens with elimination of all 'known' conditions, and a fulfilment of several symptom criteria listed in the International Consensus Criteria for Chronic Fatigue Syndrome (CFS/ME).
Finally, having a diagnosis to focus on, I began to find out all I could about the illness. Year by year, I discovered that I could fight the symptoms from many different angles. This included looking carefully at what I ate and how it made me feel, managing my physical and mental output, as well as keeping my stress to a minimum. I realised that I needed to avoid harsh chemicals and that standing too long would make me very dizzy.
So gradually, living my tightly managed life, I was able to function to about 85% capacity. But still no one could tell me why my body couldn't totally keep up with the demands of normal life.

I felt a mix of complete bewilderment and despair arriving at this place once more, but I'd 'recovered' previously, so I knew I could do it again.
This time though, I was in a different situation. I had done years of studying lifestyle, health, and nutrition and I had the internet as my friend. To my delight, I found some fantastic new research, which would finally solve the mystery of why I felt like I did.
The research being done was in relation to the Mitochondria. These little sausage shaped elements of every cell in our body are fundamentally responsible for producing the magical chemical ATP (Adenosine triphosphate). ATP provides energy to our muscles and all the busy internal organs in our body, including the liver, heart and brain. So this then raised the question of what's causing the blockages? To my real surprise, my test results showed it was due to various, off-the-shelf chemicals (found in, amongst other places, personal care products and household cleaners) along with a mix of nutritional deficiencies caused by malabsorption or diet deficiencies. In other words, modern day life was truly taking its toll!
This information about energy creation in the body all made perfect sense to me, and was why I struggled so much to keep up with everyday functions and demands. It also confirmed why the many protocols of healthy lifestyle changes, as well as good nutrition, and minimal exposure to chemicals, had produced results in the past. My slipping into bad habits, and allowing too many chemicals to enter my life had once again, gradually blocked and reduced the production of ATP in my cells. With my new knowledge to hand I could now follow an even more targeted approach to get myself back on track.
Finally, the mystery of my life was solved. Instead of 10 years, as previously, to get to the point of walking again without a stick, it took 12 months.
Reversing the effects of such imbalances in the body can be a slow process, and the lifestyle changes and nutritional needs of each person varies, but to travel with a new understanding of what was happening, gave me a sense of clarity. The open loop of the last 20 years had finally been closed.
